Community Home Care Nurse Job at Taku River Tlingit First Nation
*POSITION SUMMARY* The Taku River Tlingit First Nation Home Care Nurse, in strong collaboration with Atlin health care team, will contribute to the overall health, including mental and physical wellness of its elder citizens. The HCN possesses an understanding of the social determinants of health and demonstrates values of community health promotion, commitment to the creation of health partnerships, and community development strategies. The HCN is responsible for collaborating with the Atlin care team, provides direction and support to Home Care Workers, and oversees care planning and documentation. Respectful engagement with community members as equal partners in health service provision includes home and community visitation and group programming. *DUTIES AND RESPONSIBILITIES* · Assesses needs/strengths/resources, plans, collaborates, implements, and evaluates home care plans and services while encouraging independence, family, and community support · Provides education, capacity building and support programming for clients, families, local care team, and Home Workers · Provides confidential individual assessment, screening, equipment, and referrals to other healthcare teams or supports as needed, including the creation, maintenance, and storage of all necessary documentation and reports · Coordinates and supervises Home Care and Home Support Workers in all manners during provision of home care · Coordinates Palliative Care Team planning and provides key support during palliation in collaboration with individual and family and with clinical, family and volunteer support, and home support workers · Works in collaboration with various community health professionals in the areas of communicable disease control; environmental health and emergency preparedness; demographic specific community health; mental wellness promotion; addictions, harm reduction, and injury prevention. *EDUCATION, SKILLS AND EXPERIENCE* · LPN or Bachelor of Nursing from a Canadian University or equivalent combination of training and experience. · Interest in critical self-reflection and continual cultural safety development and training, including seeking guidance and direction from Taku River Tlingit Leadership. · Respectful of individual, family, community and nation self-determined approaches to health and well-being · Ability to work both within a team and independently · Computer skills and willingness to learn new programming as appropriate · Knowledgeable and respectful of Indigenous traditional healing practices · Cross culturally sensitive, trauma informed, and aware of the need for reconciliation practice in programming delivery · Demonstrated interpersonal, conflict management, coaching and leadership skills *CONDITIONS OF EMPLOYMENT* · Current CPR-C certification (that includes infant resuscitation) · Completion of a vulnerable persons expanded criminal records check · Current valid BC driver’s license *BENEFITS* · Included after 3 months of probationary period. · Benefits includeextended medical, eyewear, dental coverage as well as an RRSP matching plan.
