Diabetes Educator - Registered Nurse Job at Family Medicine Office

Provide nursing functions including prevention and management of diabetes and participates in implementing, monitoring, evaluating, and developing programs and services for patients and caregivers.

  • Perform nursing assessments, development of client-focused diabetes management plans, health education/counseling, and other clinical activities according to the College of Nurses of Ontario standards of practice and the current Diabetes Canada Clinical Practice Guidelines;
  • Counsel clients on insulin initiation, adjustments and pattern management;
  • Make recommendations to primary care providers on medication and insulin changes for clients;
  • Work under medical directives to assist in the medical management of patients living with diabetes (this includes making recommendations to initiate, adjust, and/or discontinue diabetes oral medications or injectables);
  • Apply motivational interviewing strategies to facilitate behavior change related to diabetes self-management;
  • Develop diabetes-related care plans based upon comprehensive needs assessment;
  • Maintain accurate, concise and confidential electronic medical record documentation;
  • Develop and evaluate diabetes education resources and counselling materials where requested;
  • Participate in meetings and educational sessions with other multi-disciplinary team members;
  • Ensures that infection control policies and procedures are followed;
  • Maintains complete and accurate patient records, assessment notes and required correspondence;
  • Safeguards patient records, assures confidentiality and privacy of patient information;
  • Contributes to programs and services cycle of planning and evaluation, and participates in centre-wide planning, committees and staff meetings as appropriate;
  • Perform other duties requested by the Office Manager

Qualifications and Competencies:

  • Current registration and in good standing with the College of Nurses of Ontario;
  • Professional Liability Protection/Insurance valid for location(s), duties, and duration worked;
  • Maintains all licenses, certificates, and standards related to the ability to practice as an Diabetes Educator and Registered Nurse in Ontario;
  • Valid First Aid and CPR;
  • Canadian Diabetes Educator certification;
  • Three to five years’ experience as a Diabetes Educator;
  • Experience working in a primary care setting is an asset;
  • Experience with foot care or having a foot care certification is an asset;
  • Able to work both autonomously and collaboratively within an inter-professional team;
  • Experience in applying evidence - informed best practices and clinical practice guidelines;
  • Experience working within a complex chronic disease framework (multiple comorbidities, polypharmacy);
  • Proficiency in the use of computers and appropriate software applications;
  • Excellent communication skills, decision-making, problem-solving, conflict management and time management skills;
  • Commitment to continuous learning, quality improvement and innovation;
  • Well developed negotiation and conflict resolution skills;
  • Ability to establish positive and proactive work relationships;
  • Reliable individual with excellent attendance record and ability to work flexible hours.

WORKING CONDITIONS:

  • May be exposed to infectious waste, diseases, conditions, etc.,
  • Interacts with patients, caregivers, family members, staff and external health care professionals where appropriate.
  • Travel may be required.
  • Manual dexterity required to use desktop computer and peripherals.
  • Overtime as required.
  • Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.
