In partnership with the Regional Stroke Steering Committees the District Stroke Program Coordinator, is accountable for providing leadership, development, implementation, and evaluation of a stroke system of care for York region based on evidence and best practices. The Coordinator will work with local partners to create and sustain an integrated regional program which coordinates the infrastructure required for a full continuum of stroke care across all spectrums (i.e., promotion, primary, secondary, acute care, rehabilitation and home care). Through collaboration and engagement amongst all stakeholders, including: Mackenzie Health leadership, Central East Stroke Network, Stroke Planning & Care Council, Ontario Health Teams, and Central LHIN partner hospitals - Oak Valley Health (formerly Markham Stouffville Hospital), Southlake Regional Health Centre, North York General Hospital, and Humber River Hospital, the Coordinator will provide strategic and operational leadership for all components of the York Region District Stroke Program. Responsible for promoting and sustaining excellence in interdisciplinary stroke care, the Coordinator will support employee learning, growth and evidenced-based practices, and will be accountable for the coordination of human and financial resources (as required) to provide quality patient care across the continuum.

• Accountable for the development and implementation of administrative and clinical policies and procedures across the region
• Maintaining, monitoring, and interpreting program reports and statistical data (i.e., clinical/financial indicators) and providing updates from CorHealth/Heart & Stroke regarding any proposed changes to funding models, metrics, or best practices
• Collaborating with key stakeholders (i.e., York EMS, Mackenzie Health Patient Flow, ED, Oak Valley Health and Southlake, Sunnybrook) to support appropriate hyperacute stroke care and achieve provincial metrics for “door in door out” and “door to needle” times across the District
• Leading the development of business cases and impact analyses for new projects and programs in collaboration with the Centre Leadership
• Providing exemplary leadership to the interdisciplinary team(s) and serve as a stroke subject matter expert
• Supporting staff development and continuing education opportunities in partnership with the Stroke CNS and Educators
• Actively participating and providing leadership on internal and external committees as appropriate including District Stroke Centre Quality Care Committee, Central East Stroke Committees, Stroke Planning and Care Council, and Ontario Stroke Coordinator and Regional Stroke Committees and planning sessions
• Chairing, Co-Chairing committees as required
• Facilitating the implementation of accreditation standards to meet Accreditation Canada Stroke Distinction requirements
• Coordinating internal and external resources to facilitate patient care and advocates for patients, stroke survivors and staff
• Providing leadership with stroke related quality improvement initiatives and other special projects (i.e. task forces, workload measurement, staffing, education and orientation of students)
• Committing to lifelong learning and continually seeking ongoing education to maintain clinical competencies

• An Undergraduate degree in a health-related discipline
• Membership as a licensed member in good standing with a professional college and membership of RHPA
• Minimum of three (3) years of recent experience in Stroke clinical care achieved on a medical/stroke device
• Experience with stroke management and familarity with the continuum of stroke care
• Demonstrated clinical practice knowledge, with previous experience in research, program development and evaluation
• Ability to discern relevant information to make effective and timely decisions regarding patient care services
• Demonstrated ability to create and coordinate inter-professional teams
• Sound conflict resolution skills
• Exemplary leadership, interpersonal, and communication skills
• Valid Ontario Driver’s License with access to a reliable vehicle required for travel hospitals in York region
• Proven ability to facilitate ongoing staff development through mentoring and coaching skills
• Proven ability to work independently, take initiative and prioritize work
• Ability to assess, interpret and project budgetary needs
• Proven ability to work within a computerized environment using Word, Excel, PowerPoint, Access and Outlook as well as demonstrated knowledge of data collection, use of data bases, data analysis, budgeting and evaluation
• Proven attendance record
• Excellent written and verbal English language skills
