SUMMARY:
KEY QUALIFICATIONS:
- A graduate of an accredited School of Occupational Therapy.
- Member in good standing with the College of Occupational Therapists of Ontario.
- Membership in O.S.O.T. and/or C.A.O.T. preferred.
- Experience in the care of older adults and management of patients with complex geriatric rehabilitation needs
- Excellent interpersonal and communication skills and demonstrated ability to work as part of an interprofessional team.
- Possesses excellent oral and written skills.
- Proficient in the use of Microsoft Word, Power Point, Excel and Outlook.
- Satisfactory performance and attendance required
RESPONSIBILITIES:
- Lead/facilitate group education programs to improve daily function in context of falls prevention.
- Assess older adults with a variety of physical/cognitive impairments and disabilities and provide goal-oriented treatment interventions in collaboration with interprofessional team.
- Conduct follow-up phone calls with patients/families, as required.
- Assist in collecting program data.
- Identify and link patients to community resources after discharge from the program.
- Participate in program planning and development, continuous quality improvement initiatives and evaluation.
