POSITION DESCRIPTION:
The Diabetes Education Network (DEN) Nurse is a resource and support for clients with Diabetes. The DEN Nurse completes assessments of clients in conjunction with the dietitian, assists with initiation of insulin as ordered by the physician, completes documentation and provides follow-up appointments and referrals as required. In addition, the DEN Nurse provides information on diabetes topics relevant to client needs in group programs or individual sessions, as well as providing in services and consultation for other health care professionals in the community and Hospital.
QUALIFICATIONS:
The successful candidate will have:
- Current member of College of Nurse of Ontario
- Certificate in Diabetes Education and Care preferred
- Clinical expertise in the management of diabetes
- Member of the Diabetes Educator Sector of the Canadian Diabetes Association preferred
- Demonstrated ability in educating adults
- Demonstrated ability to facilitate group and individual educational sessions using the principles of adult learning and the process of assessing, planning, implementing and evaluating the educational program
In addition, the successful candidate must be available to work day, evening and weekend shifts as required. An ability to attend work on a consistent basis is also a requirement.
Job Types: Part-time, Fixed term contract
Salary: $34.24-$49.02 per hour
Schedule:
- 8 hour shift
- Monday to Friday
